All relevant forms (e.g., intake paperwork, informed consent, HIPAA privacy practices, release of information, etc.) will be made available through your established client portal. Prior to your first appointment, all applicable forms should be completed. Completion of the forms should take no more than 10-15 minutes, and completed forms are confidential and can only be seen by Connect staff.  If you prefer completing paper forms, please arrive at least 15 minutes early to your first appointment to complete these at the office.
